In accordance with Internal Revenue Service (IRS) guidance, Cooperative Home Care, Inc. will not automatically mail a copy of your Form 1095-C (Employer-Provided Health Insurance Offer and Coverage) for the 2025 calendar year unless you request one.
Your Form 1095-C provides information about the health coverage offered to you during the prior calendar year. You may request a copy at any time.
