Supporting People with Disabilities
We understand complex care. We provide thorough classroom and hands on training for our Care Team including Advanced Personal Home Care, and Crisis Prevention Intervention.
Cooperative Home Care is an approved, contracted provider with the Missouri Department of Mental Health (DMH).
The Lopez Waiver Program, MR/DD Comprehensive Waiver Program, MR/DD Community Support Waiver and the Home and Community-Based Waiver Program all require the following eligibility:
- Developmental disability incurred prior to the age of 21.
- Medicaid (MO HealthNet) eligible.
- Would otherwise require placement in a facility.
- Other criteria may apply for individual programs.
Services provided under these waivers are provided as an alternative to the level of care in a facility or to allow an individual to be discharged from the facility into the community.

Services provided by Cooperative Home Care for our DMH-funded clients include: • Personal Assistant
- Personal Assistant – Medical/Behavioral Challenge
- Developmental Habilitation Training
- Nursing
- Attendant Service
Cooperative Home Care is one the largest and most experienced independent providers of in-home services for the Missouri Department of Mental Health in the St. Louis area.
What kind of support do you provide?
We provide Department of Mental Health (“DMH”) services. We also provide Department of Health and Senior Care and Consumer Directed Services. Cooperative also provides Pediatric Nursing Services through Missouri Medicaid and other health insurances.
Do you help people who are younger than 65?
Yes! Cooperative serves older adults and people of all ages with disabilities. We even provide pediatric nursing services. The most common way we support younger people with disabilities is through our Department of Mental Health services.
