There’s no place like home.

Our founder, Fran Sontag, was driven to help people who were often overlooked and who could not advocate for themselves. She began by providing in-home education for several young adults with disabilities. Through serving them, it was apparent that they could benefit from additional in-home services. Fran’s drive and dedication continue through her son-in-law, Mitch Waks, and through all of our employees today.

What We Do

Any Funding, Any Amount of Care, Any Skill Level

We make it possible for people to maintain their independence and rehabilitate at home. That means:

Any funding. We explore and provide any possible funding source, from government (Medicare, Medicaid and VA) to insurance (health, long-term care and worker’s compensation) to reduced-fee programs and self-pay.

Any amount of care. We have no minimum or maximum hours. We offer visits, hourly care and 12- and 24-hour shifts.

Any skill level. We provide nursing, therapies, social workers and caregiver services.

Dementia Speciality Program

Cooperative has Certified Dementia Practitioners who are accredited through the National Council of Certified Dementia Practitioners (NCCDP). Our trainers provide dementia education for caring for people with dementia during our two-day orientation training, as well at our annual caregiver in-services. We also offer Crisis Prevention Intervention (“CPI”) training for caregivers which helps them proactively and safely care for individuals who may be confused and/or agitated.

Free Assessment And Care Plan

Our free assessment is a great resource even when you are not quite ready to begin services. You can still discuss the specifics of your loved one’s needs, learn about how to pay for home care and what it may cost you, and begin considering your loved one’s options. At the end of the assessment, your completed Communication Book is ready, including your customized care plan.

Our Proven Process

Cooperative uses our unique ability to accept every type of funding to provide the most comprehensive and affordable home care services. We begin by qualifying you for every type of home care funding. We help you access all of your existing benefits through MO Medicaid, Medicare, long-term care insurance, and VA funding. Only then, if needed, do you pay out-of-pocket.

We Are Your Most Affordable Option

We explore and provide any possible funding source, from government (Medicare, Medicaid and VA) to insurance (health, long-term care and worker’s compensation) to reduced-fee programs and self-pay. You will not pay out-of-pocket until we have exhausted every other resource.

What People Are Saying About Us

We were very pleased with Cooperative Home Care and feel fortunate to have had it. Theresa and Larry were excellent and we appreciate having them. Thank you!
The people I had were not only very thorough and professional but also cared about me and was fond of all of them.
Thank you so much. All of my support was extraordinary. A special thanks to Sanita and five stars to Mary Allen. She is definitely an extraordinary professional who deserves much needed praise and help for taking care of Mom.
Arnetta S., Ann C., and Kimmi B. are three angels who were dropped in our lives to take care of Mom!
Our worker, Jeannette M., was outstanding and took exceptional care of my mother while she was with Cooperative Home Care, and I would recommend her services to anyone I know!

We have contracted with your company for about five years. We have had a consistently good experience with no problems. Your aides and supervisors have been very helpful and supportive to S. and myself. We have become family. Your Cooperative Hospice was required for the last several weeks. They performed well beyond my expectations. It would have been a very much more difficult journey without their help. Please extend my special thanks to: Ellen B., Kathy P., Shirlean B., Sandy T. and Donna C. Thank you for your leadership and care. You are providing a major service. I have recommended your company to several of my friends and will continue to do so.

We just wanted to put in a good word and a show of appreciation for Patrice and Sheila, our workers, during this recent spell of bad weather. These women made it each & every day and we are very grateful. Many thanks to each of them.

We shall always remember with deep gratitude your comforting expression of sympathy. Thank you so much for the beautiful plant. It has made a difficult time easier.
- The S. Family
We have used your fine services since February 23, 2015 for A. and are grateful for the most difficult works of the caregivers. Most of the time, (Caregiver) Annie R. took care of A. during the day and she was wonderful. Annie, myself and A. developed a fine relationship because of the professional care Annie rendered to A. as well as a devoted personal association. (Caregiver) Gail C. assisted A. on occasion and she, too, was an exceptional caregiver. I would recommend both for any job potential. Your firm also sent a lovely bouquet of flowers in memory of A. and we are also so appreciative for this kindness…Thank you.
- S.
They were wonderful to my friend and her family during a very difficult time. Bless you!
